To enroll in any of Center Point DAAC's services you must complete our easy screening and assessment process. This process ensures that you receive the appropriate services based on your individual needs.

If you are an agency, enrolling a client in custody, or on their behalf, do not use this form. Call 707-360-4399 for assistance.​

​The new client enrollment process is as follows:

​

1. Complete the Client Services Request form below. 

​

2. An Admissions Counselor will call you. Please be sure to answer calls from Caller ID: CP DAAC (707) 544-3295. The Admissions Counselor will review your information and schedule an admissions appointment.

​

3. You will complete your services screening, assessment, and if accepted into treatment, you will complete all of your new client paperwork and start services. Submitting this form is only a request for services, this does not automatically enroll you in services and does not constitute acceptance into treatment.​

​

We also offer Open Enrollment. Open Enrollment is every Tuesday from 10 AM to 7 PM at 2403 Professional Drive, Suite 101, Santa Rosa, CA 95403. Clients are enrolled in the order in which they arrive.

Our residential treatment program is for men only. All other programs we offer are for both women and men. For women's residential treatment services, please visit: Women's Recovery Services or Athena House.
