Request Services

To begin your enrollment in any of Center Point DAAC's services you must complete our easy screening and assessment process. This process ensures that you receive the appropriate services based on your individual needs. This process starts with the form on this page! Please complete this form before you come to our office.

If you are an agency, starting the process for a client in custody, or on their behalf, do not use this form. Call 707-360-4399 for assistance.​

​The new client enrollment process is as follows:

​

1. Complete the Client Services Request form below. 

​

2. After completing the form below, come to our office at 2403 Professional Drive, Suite 102, Santa Rosa, CA 95403 between the following hours:

Enrollment is on a first come, first serve basis. Upon arrival, you will be added to the list of enrolling clients and an admissions counselor will meet with you as soon as possible. If you want to be first on the list, the best time to arrive is before 10 AM but no earlier than 9 AM.

Mondays: 10:00 AM to 6:00 PM

Tuesday: 10:00 AM to 4:00 PM

Wednesday: 10:00 AM to 6:00 PM

Thursday: 10:00 AM to 6:00 PM

We do not admit people to services on Fridays, Saturdays, Sundays, or Federal holidays.

3. You will meet with an admissions counselor to complete your services screening, assessment, and if accepted into treatment, you will complete all of your new client paperwork and start services. Submitting this form is only a request for services, this does not automatically enroll you in services and does not constitute acceptance into treatment.​​

Our residential treatment program is for men only. All other programs we offer are for both women and men. For women's residential treatment services, please visit: Women's Recovery Services or Athena House.
